INSURANCE

We will bill your insurance as a service to you. We are not listed as preferred providers for any insurance plans, however some programs allow participants to see providers outside of their programs for a lesser benefit. Any balance left after payments by your insurance company are your responsibility. It is in your best interest to understand your own insurance plan. We are currently not accepting any new Oregon Health Plan patients.
PAYMENTS

Payment is expected at time of service unless prior arrangements have been made. We accept payment by Cash, Check, VISA, MasterCard or Discover. Credit card payments may be made by telephone.
APPOINTMENTS

We request that any appointment changes be made with at least 48 hours notice. Repeat last minute cancellation and missed appointments will be charged. We understand legitimate emergencies do occur. Any patient that misses three appointments may be dismissed from our care.
FINANCIAL ARRANGEMENTS

Special arrangements can be made to extend large balances over a period of 90 days. These must be made prior to the initial treatment.
